Case description
This patient was referred by another dentist for a treatment of his mandibular 2nd molar, following a pulpotomy, due to the irregular anatomy of the root canal system.
Despite the pulpotomy, irrigations by NaOCl 5.25% and antibiotic therapy, the patient complained of sharp pulpitic pain. The initial radiograph revealed what looked like a C-shaped canal configuration.
Following isolation with a rubber dam, investigation of the pulpar floor began, with an ultrasonic device, and indeed, a C-shaped configuration (Fan's C2b class) was found, as can be seen in the clinical microscope photographs.
The initial radiograph gave a hint on the presence of a delta formation in the apical area. Cleaning with the SAF System (4 minutes with 5.25% sodium hypochlorite) allowed to clean this complex configuration. An irrigation with 17% EDTA was done, and was followed with some more NaOCl.
The root canals were dried with alcohol-soaked paper points.
Obturation was carried by Thermafil cones - one in the ML canal and two in the C-shaped canal. The use of the heat-softened GP allowed to reach the apical delta and to adapt the obturation to this irregular morphology.
