Case description
This male patient was referred to the Maccabi-Dent dental clinic in Netanya, with a complaint of local pain from a mandibular 2nd molar (#37).
Upon check-up, there was pain to percussion and palpation, and the tooth was found to be non-vital. The initial x-ray showed apical periodontitis focused on the mesial side of the tooth.
An access cavity was opened, and the pulp was found to be necrotic. 4 minutes of operation of a self-adjusting file 1.5mm were conducted in each root canal, and the canals were packed by calcium hydroxide for 2 weeks.
On the second meeting, the calcium hydroxide was cleared by SAF 1.5mm, and the tooth was obturated by a combination technique (lateral compaction at the apical area, followed by warm vertical compaction). The penetreation of the AH+ sealer is noted into a large mesial lateral canal.

Dr. Ben-Yitzhak is the deputy director of the Post-graduate Endodontic program at the Sheba Medical center in Israel.
He graduated from his dental studies in 1991, and graduated from the Endodontics Specialization program at the Hadassah Faculty of Dental Medicine, Hebrew University of Jerusalem in 2003. He served in the IDF for 17 years in various positions.
