Case description
The patient arritced to the clinic with this mandibular 2nd molar (#37), diagnosed with chronic apical periodontitis. The initial x-ray photo showed suspicition of an irregular canal morphology, and the patient was sent for cone beam CT. The CBCT confirmed the suspicion - a C-shaped canal that divides into several separate canals in the apical area.
The self-adjusting flie system was used to treat this case, by adapting to the irregular anatomy of the canal, and was used with continuous irrigation of 4mL/min NaOCl. The SAF was used in different areas of the canal, to allow it to instrument all parts of it, while focusing on the different exit points.
Obturation was done by warm gutta percha, and the existance of lateral canals is also demonstrated in the final x-ray photo.
